Does your airline allow you to express fatigue about the other crew member? How can you diagnose fatigue? Or, how about professional standards? Ever thought approaching them? You're right, you're a pilot, not a doctor. You can state your observation first with Bob... "Hey Bob, I've been noticing a pattern where I had to intervene/correct. You fell asleep on short final - if I hadn't taken the airplane, we'd have ended up in the news." Then take the next steps... ProStan and start from there. Express your concerns and encourage others to do so as well.
This is not much of a different conversation, although a little harder on the old guy, than you'd have with a millennial or Gen Z'er whose cellphone habits are indeed a safety concern, and when confronted about it, complies for a leg then conveniently forgets to the following leg.

Your reply to me changed the topic from cognitive decline to an example of fatigue. I agree with what steps should be taken in your fatigue example. No, my airline does not have a process for me to call in fatigued for the other guy...and no, this is or the cognitive decline of other pilots are not issues I normally have to deal with because I'm a higher seniority Captain. The cellphone habits, on the other hand...

Imagine if the only FAA medical certification change from raising the age is that the FAA is permitted access to our electronic health records from every medical visit we have ever had.
Well, the FAA is already studying just that.
Do the old guys care? Hell, no. They'd welcome the excuse to go out on disability for two extra years. But it would have a lot of collateral damage.

The only way to have a proper medical monitoring system is to have the most accurate information and a reasonable action on concerns.

Whaaaat? That's a keeper! I've had one AME's office call me up two days later as the secretary noticed that I forgot to list a colonoscopy appt 3yrs prior on the most recent medexpress that had been previously listed.

This is the most frustrating part about being a unionized airline pilot. Since about Reagan the pilot group has constantly leaned towards putting individuals in power that continually opposed labor, and then they're shocked when their union has no teeth or influence. They love big business and CEOs....oh except the ones they have to work under. They speak awful things of other unionized professions, but but we are different! History repeats and we have no one to blame but ourselves.
Fortunately, I believe the major CEO’s and unions are in agreement on this one. That should speak volumes.

Some want 67, but think we’d come to that without any further testing even though demenia/cognitive decline is escalating in this age range? Amazing how such safety oriented thinkers are suddenly happy throwing caution to the wind when there’s $ in it for them.

Heard the same thing when they were looking at upping it to 65. No real difference here amongst the con arguments. 67 is going to happen, or not, despite what "we" think. The national regulatory agencies will look at it and see what, if anything, they need to change med cert wise as a result should 67 come to fruition.
