At Flight Safety International I was tasked with developing a two hour "Pilot Monitoring" course that was mandated by our CEO after an inquiry from the NTSB.
"Failure to Monitor" has been a causal factor for some time now--one of the cases studied was EAL 401--but the focus these days is on training the "monitoring process."
I spent about six months developing the course and delve deeply into the "why humans are not very good at monitoring" aspect.
