I was correct in my previous post about the information printed on the backside of the jumpseat request form. I just rode back home in the cockpit jumpseat on company mainline metal, and kept the pink "jumpseat traveler" copy in my posession so I could restate what is printed on the form. At the very bottom center of the jumpseat form it says:

"See DRS:JUMPSEAT, DRS:CABIN JUMPSEAT or the US Airways FOM for expanded guidance."

DRS = Direct Reference System which is where the gate agents can access and read a help file on a particular subject in the SHARES reservations computer system that US Airways uses.

I always dress nice when I travel. Anyone on my passes dresses nice. Who cares what the policy says. If I worked in an office I wouldn't show up there in my flannel jamies. I won't do it at work here either.
