I asked our POI today during my .293-.297 check ride and he did not know either. He basically said if your clearance limit is the airport and you filed to the IAF and then the airport for that particular approach and the controller is that brain dead it sort of falls under lost comm's. Obviously the controller would have to have fallen asleep and no one noticed so the issue would probably only be the initial turn after the IAF instead of holding while the pilot maintains the last assigned altitude until cleared or declares an emergency. If he falls asleep late at night just follow the lost comm's procedure.
It is a little bit of an over think excursion which I have been known to do from time to time.

If you ask ATC and they don't answer that's obviously lost comms, do what you need to do...just make sure your TCAS is on.

ATC can sometimes assign you to "Track outbound/inbound" on the approach once reaching the IAF without explicitly clearing you for the approach. You will most commonly see this when going into an uncontrolled field (US) where they are withholding an approach clearance due to departing IFR traffic, but don't anticipate a delay in your arrival. Since they can't explicitly clear you for the approach, many controllers will have aircraft follow the published approach (full procedure) but only clear the aircraft once the departing traffic has been positively identified and one-in, one-out is satisfied. I've received approach clearances as late as 1nm outside the FAF in such situations before.

During the controller strike,[many closed towers] center would give you a vector to the final app. course and tell you to do your thing, full app. p.t., everything on the page.call in the blind on tower, and contact flt. service for reported traffic. Very, very, lonley in the dark of night.Click, click lights on,oh no, the the rabbit on full bright!

This is what had me confused. I have had allot of these. Past the IAF PT out or inbound, told to report it and then cleared after all the while descending on the PT out/in.
