It seems like the results in this "analysis" might have been obvious the very first day.

Is the crew still employed? Do they still have their licenses?

This is why its best to back up the visual with a precision approach and brief the runway lights as well. Sounds like they got a pass on this incident due to the faulty avionics plus a nice Chief Pilot.

Depends on the airline, they're particular culture/management, etc. Often times, it's NOT the CP's decision to ACTUALLY fire pilots for whatever happened. The decision itself can come from higher, with the CP simply being the hatchet man.

Besides, companies (especially with representation) are laden with CPs firing pilots willy nilly, only to have that reversed. And usually, because the CP was an idiot with a "fire friendly" mentality.
