Ok, this is a first for me. OMG! I'm trying get a reimbursement from my Limited Purpose FSA for two separate crowns my wife got this year. I mailed in the first claim with copies of both EOB's. I get the response that my claim was denied: "The information you sent is unreadable, illegible or a corrupt file. Please resubmit." So the next time I submit my claim electronically with photo's of the EOB's. I just got the same denial reason! Are you kidding me! They can't read a copy of EOB made with a copy machine or they can't look a picture of the same thing? Crowns aren't cheap and I don't want to forfeit money over the $500 carryover allowed.
This is total BS! This is MY money, NOT UHC's! My appeal letter is going out tomorrow. I'd call but of course they don't provide a phone number......the rat bastids!
I've never had a claim for reimbursement from my LPFSA denied before. Un ******* believable!
Anybody else have/had this problem?

I can't help but it sounds like total BS. UHC is a terrible company. They once referred me to a clinic to save money. Turned out that clinic was owned by a hospital and therefore just as expensive. Filed a grievance and they told me to fly a kite...

The customer service number is on the top left on the first page of the claim form. I’ve never had a problem getting them on the line. I have had a couple of claims denied for no apparent reason. Both times I called, they resubmitted and were approved. Must be a random denial generator somewhere in the works.
Where are you sending it electronically? I didn’t even know that was an option. I’ve always just faxed it according to the directions on the claim form. A process firmly entrenched in the 80s.

The UHC website allows one to e-file HSA reimbursement forms. Scan the receipt, fill out the form on the webpage (it should only take 2-3 minutes tops), attach the scanned receipt per their link, and hit enter.
I’ve been doing this for multiple years with no issues. I usually circle the pertinent data on the reciept (name, date, service description, amount, etc.) to ease the process.

Because I have tricare and not UHC insurance I couldn’t even establish an account for login. Just been doing the old fashioned Fax method. Would love to be able to submit online though.

Another thing that I do when submitting the claim is I include the EOB and the actual bill I paid. I've had it denied just submitting the bill since they claimed it was a duplicate submission covered under other insurance.

Rooster, you should reapproach. I use tricare and the medical FSA for the last couple of years and submit all my "expenses" on the online form and lately all I submit is just the Tricare EOB and they reimburse my "patient responsibilty" portion on the EOB without any other bills, etc.
