let me guess. You’re a direct entry captain who got downgraded and no chance any of these events would have happened with you in the left seat.

A lot of guys are going to get defensive and fingers are gonna start pointing, with assumptions of finger wagging from other pilots here. The FAA realizes the problem(s). There are some that are hard to mediate, such as lack
of flying due to Covid. Culturally though, Eaglevoy has become a land of “foot on the neck” management. Steep veiled threats for everything from wearing the wrong pants, to more importantly, ON TIME performance. Rush rush rush. Safety comes second because if you can’t do it on time it’s a failure on the part of your job. That is the insinuation. FAA has seen it a million times. Almost all of these airmanship issues could be solved by slowing the efff down. The second you feel rushed, smile and slow down. For safety. Then when you get a carpet dance, or even an email, you send it on the FAA whistleblower hotline saying that you were pushed to not slow down and be safe.
So to sum up, the FAA just put the onus on you pilots to make sure the finger gets pointed in the right direction. Don’t let the company pass the blame. If you have wieners for union leaders, replace them with non-succubus leaders. The company needs a serious pilot leadership change. It’s been too long and they will never get a fresh perspective for change with the current leadership. Empty “safety first” emails backed up with the same blame the pilot culture will result in the same results.

I am not going to defend or attack any of this, as I wasn't there. It would seem a lot of this has to do with management forcing upgrades on low time pilots, Covid and ATC, however, Envoy does have a no fault go around policy among others. I have never felt that if I slow down and be safe management would get on me for it. I have always believed that if safety guided my decision, Envoy would support that.
I just think this boils down to experience, currency and perhaps too much trust in ATC reporting. Often times those runway condition codes are hours old and if you miss that key bit of information, you can get in trouble quickly.

Thanks for sharing, any chance you have the Event 1 as well?
I remember even 5 in ORD as i just landed a few minutes prior to them. I elected to land 9R as they just opened and said it was 5/5/5... which it was not.
