The granting of ETOPS depends on a number of things. The government and agency granting certification, the people running the company, the experience of the pilots, the maintenance program, and the aircraft in question.
In the case of the Boeing 737 NG's and some 777's they all come from the factory pre-certified with a maintenance program fully developed and approved (the duration being a function of the above and how many batteries are installed during assembly). The NG has the capability to be up to two hour certified immediately if the pilots and management meet the criteria and the operator orders the plane with two batteries.
The Airbus Boeing 767-300's don't come certified and an operator must apply for certification for the aircraft as well as the personnel and programs.
Kalitta has no maintenance program, no experience in ETOPS operations and us pilots that have prior experience, have not done it in a long while. Unfortunately, it will be the long road for K4.

The rule change in 07 opened the door for a very expedited process. I.E. Atlas, who contracted a known consultant and subsequently got it done chop chop.

Is this a build it and they will come strategy or is there something specific in mind, any idea?

Nobody knows what Connie has in mind. Could be DHL, Ad Hoc, additional lift for PAC, Middle East, or if the price goes high enough on 76 Freighters to give him a nice profit on the conversion he could sell them.

Considering how godawfully short we are for the 747 right now, I don't hold much faith in them suddenly being "properly staffed" for any new planes anytime soon.

A new contract with segment leading wages and benefits and we will have all the staffing they need. Between YIP and DEN we can train 16 at a time. 767 training outsourced to many many providers. Training is not the issue, putting bodies in the training slots is the problem.

Fortunately, pilots are showing their objection to our contract with their shoes. Many pilots gone so far this year and lots of empty interview chairs. For once the playing field is starting to be leveled. The RLA is a disaster for pilot groups.

Which "Worldwide" are you referring to? I know of 3 aviation holding groups with "worldwide" in their name. Serious question.
