Yes you can fail AQP.
MV: Run out of time to retrain.
LOE: Crash, or create serious doubt as to safe outcome. Limitation busts might be a debrief or might fail.

In my opinion, it has very much to do with one particular instructor who was told to stop pulling this crap. I know of a very senior captain, a lifer, who walked out on this instructor. This is normally a calm, level-headed captain that I'm talking about who couldn't take this guy's crap any longer. They will continue to lose good pilots if they don't do something about this guy!

For somebody who doesn't even fly (according to your moniker), you sound pretty cocky! If you do happen to be with Mesa, my wish for you is to get the instructor that I speak of, and then we'll see how you feel.
