NTSB Investigates Runway Incursion At CLT
Did you guys hear about this?!! Sounds like some one is in deep ******.
PC-12 v CRJ-200 Altercation Was Almost A Disaster
"The National Transportation Safety Board is investigating a runway incursion that occurred on Friday morning at the Charlotte Douglas International Airport (CLT) involving a general aviation aircraft and a regional jet airliner bound for New Bern, NC (EWN). At about 10:17 a.m. on May 29, a PSA Airlines CRJ-200 regional jet operated as US Airways Express flight 2390, was cleared for takeoff on runway 18L. After the regional jet was into its takeoff roll, a Pilatus PC-12, a single engine turboprop aircraft, was cleared to taxi into position and hold farther down the same runway in preparation for a departure roll that was to begin at the taxiway A intersection.
After the ground-based collision warning system (ASDE-X) alerted controllers to the runway incursion, the takeoff clearance for the regional jet was cancelled.
The pilot of the PC-12, seeing the regional jet coming down the runway on a collision course, taxied the PC-12 to the side of the runway. The FAA reported that the regional jet stopped approximately 10 feet from the PC-12. Visual meteorological conditions prevailed with 9 miles visibility. There were no reported injuries to any of the 42 passengers or crew of three aboard the jet, or to any of those on the PC-12."

WOW! Just yesterday at my home field I was sitting number 3 at the hold short and the tower controller cleared a flight of two for takeoff while another aircraft was on short final (probably less than 2 miles). Luckily - a supervisor came on and canceled the takeoff clearance for the flight of two. We have lots of trainees at my field too. HEADS UP!

How about a flight of 4 cleared onto hold in front of a DC-9 on short final. Luckily the DC-9 crew saw the 4-ship and executed the go around. Everyone gave the flight lead c**p for not seeing the DC-9 as he was waiting for t/o clearance - the common theme was that he had his mirrors positioned so that he could admire himself rather than see some big airplane trying to land on his flight.

We saved a controller's bacon at Orange Couny SNA last year...
He cleared us to cross the short runway 19L and hold short of 19R... we were about to cross when I noticed a C-152 on short final for 19L, so we held our position.... ATC said afterwards "Thank you QS"
we help them out when they screw up and they do the same for us (most of the time.....)

A glance up final is ALWAYS important, but especially internationally. In Caracas one time we were cleared to pos & hold, looking right up at a herc on 1/2 mile final. I've always wondered if the controller thought the herc was so obvious, he didn't need to point it out. It was obvious to us, at least.

Happened to us in AUS Rway 17R on a short final in a CRJ-900.
We were cleared to land; Southwest 73 was holding short off 17R and called in that they were ready to go. Tower cleared 73 for takeoff without even mentioning that ‘plane on [mileage] final; Southwest boys/girls apparently NEVER even looked at the final otherwise they wouldn’t have pulled on the runway (or may be they would’ve).
We couldn’t believe it. We initiated a go around and it was at least few seconds before the tower realized what happened and simply told us to turn right heading 190. Unbelievable!
It seems that in AUS, tower/ground would do whatever they could to help Southwest get out of there by allowing them to cut everyone and go in front. We once waited for TO on 17R at the beginning of runway and they had us hold for five minutes because the SW 73 was taxiing in and was allowed to TO at G. We saw a massive dust bowl rise in the air at the end of runway 17R with an almost abrupt ‘liftoff’.
Oh well. Always watch and decided for yourselves if it’s OK to move on to the runway or when landing, make sure no one’s on it.
