I know the question asks about the most challenging approach domestically but LOL @ the Expressway Visual 31 LGA or the DCA 19 approach. Try Kathmandu, Nepal...

I have also heard great things about the Kai-Tak Heart Attack on the old Rope Starts and 400s into the old Hong Kong airport. Apparently it could only be flown by first officers, since it required an insane right turn at a low altitude going base to final. Don't know how true that is, but those are the stories I have heard.

From what I’ve seen if you are a career wide body bunkie, any visual approaches are the most challenging.

My Vernon vis 1 circle 33 @ DCA. Key West is another fun one.

In the lower 48 in 121 jets :

- Aspen by far.
- DCA is not hard but a little more stress due too someone living there.
- PWM by far the most fun and beautiful. Click everything off over the ocean and start doing your thing. Last turn inside the marker.

Expressway visual to 31 was fun.

After they took the tanks down it was fun to come across the Verrazano on the RNAV and tell approach "United xxx has the Masspeth Tanks insight" ... One guy actually laughed and cleared me for a visual.

Not true. The guys who did it all the time had no problem, when it was windy some of the ME carriers put on an air show.

The Quiet Bridge Visual into the 28’s into SFO was neat to fly over the bay and checkout the views back when I used to fly in there a lot. Not sure if they still have that visual approach anymore. Wasn’t challenging though...
