Keep trying different depts till you get the right one at FAA. They should be able to tell you if a request was recd. - I also believe they can send you a receipt stamped with the date your records were released. Be persistent with FAA- they should have your answer.

Hey guys/gals,
I'm in the middle of applying for the Blue. Unfortunately I failed my private pilot check ride in 2006. In order to disclose this, JB wants documentation. I have none. Does the FAA keep records like this, and will they give me access to them?

You can request your entire record history from the FAA here:

They will have every piece of paper ever issued with your name of it for FAA purposes.

How are check ride failures at the initial level (Private, instrument, CFI etc) viewed by JB? In other words are one or two (or more) at that level of our careers just as a big deal as a 121 failure? Or is a check ride failure at any level weighted the same?

It's not that big of a deal as long as you can show that you learned from the situation. I have failed 3 rides, 2 at initial level and 1 at 121. 2 were orals one practical. I just finished background check and am waiting for phase 2 complete email. Explain them, what you learned and how you have progressed you will be ok. I have a friend waiting on a class date that failed his ATP ride. It's all about you and how you handled it. Good luck.
