Just so we can understand a little better, how far in to the sims did you make it? How did the IPT's go?
Really, what were the major issues that you had? I know that you said it was staying ahead of the airplane, but can you be a little more descriptive.
You obviously had a bad time of it, and maybe posting a little more about it will help others realize just what you need to be prepared for when you get a job with a regional airline.

The FAA didn't seek enforcement action. But yeah lesson learned from that.

At PSA I had a terps issue. I had trouble correlating SOPs. For example on my last sim RVR was 1200 and I elected to continue the approach.

Yes, I've seen mine. You could order them directly from the FAA.
Turnaround times aren't the fastest, at least they weren't last time I ordered them for myself (5-7 years ago), but they are available.
Keep in mind, inconsistencies in one's pilot application and actual PRIA records are cause for termination at any point.
If you don't mind me asking, why do you think you were unsuccessful in passing initial training at two different companies?
If it was for personal reasons, did you have something going on in your personal life that was distracting you?

Please stop trying to fly airliners for a living. There are plenty of solo pilots jobs out there where you are less likely to kill anyone. Look in to ferrying airplanes.

Well, it seems as if this thread is filling up rather quickly, so I'll help with that and throw in my two cents worth, hoping this isn't troll bait. Every so often, the corporate world and the FAA have had a back to basics campaign when it's discovered that is exactly what is needed. If memory serves correctly, the FAA had their last back to basics emphasis about twenty years ago. And it was named exactly that.....BACK TO BASICS. I consider myself fortunate in that I had and old WWII Army Air Corps guy who had been a pilot/instructor. I got some great dual on basic instrument skills and looking back on it now, that was what got me through when I was lacking in other areas. Especially when I was very good friends with Johnnie Walker and his gang. Maybe an evaluation of basic instrument skills would be in order. And then there's an evaluation of basic life skills, like attitude, and such. And that may be much harder to detect and judge.
