Based on what I’ve read I wouldn’t call the Captain weak.
He never failed the ride, he wasn’t recommended and he passed after he was.
A very recent upgrade with 157 hrs PIC on the 767 basically means 2-3 months as Captain.
Then he gets paired with somebody who apparently had a fatal flaw in his skills.

PRIA must be useless, or airlines are just ignoring it and play dumb. I'm astounded he got hired at Atlas with that work record, they must be a lot more desperate than I thought. Ditto for Mesa. I hope this gets picked up by the media, maybe in conjunction with a story about the pilot shortage.
I'm also curious about the work records of the pilots involved in the runway overshoot at NAS JAX. That Captain with 13 years made about the same as a 3yr F/O at Delta.

Why aren’t the companies required to send info to a central database so potential employers just pull from there instead of requiring the good will of an applicant to be honest with prior employment?
This is a systematic failure exploited by someone who wanted a career as a pilot and lied as a result. Just wow.
As someone else previously said, this will happen again.

The A/P wasn't engaged. I don't really care what the report says... It's obvious that the A/P wasn't engaged from what happened and from the video where you can see the jet pull up.

Employers were directed years ago to start saving relevant data for future inclusion so when it goes operational it should go back about ten years. Although it will probably be more complete for records created after the date of implementation.
Caution: the airman records are not necessarily complete (mine are not) and accurate in PRD, so just because something is missing in PRD do NOT assume an employer can't get it via PRIA or FOIA.
I'm guessing this situation will lead to an expedited full implementation of PRD

It was mandated by law after colgan, it's called the "Pilot Record Database" and it's only partially functional to date. Your FAA records (specific stuff, not everything) are available but the employer records part has not been implemented yet.
Employers were directed years ago to start saving relevant data for future inclusion so when it goes operational it should go back about ten years. Although it will probably be more complete for records created after the date of implementation.
