thre's a lot of places in the world where everyone wearing a pilot costume is "Captain".

Yes, and they include US airports. After my second or third time during my first week as a newhire, I gave up on correcting them and accepted it as normal from ramp crew, fuelers, baristas, etc. But not when FA's and gate agents did it, they should know better.

And places where actual captains/first officers are mistaken for and called bell boy, sky cap, etc...

Yep. I think that's an overlooked revenue stream ... hanging around the hotel lobby in your uniform and collecting tips for carrying luggage.
