For CFIs or people flying piston airplanes it's pretty beneficial to get an hour or two in an FTD and set it up like a turboprop to practice flying the hold at 160-180 and the approaches at 130.

Can someone tell if one needs the actual ATP to interview or just the written? Also, is the 135 written ok, or does it need to be the 121 written?

Just the written. It doesn't matter if you took the 135 or 121 test.

I would guess not, I was furloughed 121, hadn't touched a plane in 18 months and I didn't do the sim. I think if you have prior 121 they assume you can fly.

I would get some prep in the most unstable FTD you could find, like rcfd said fly it at 160kts. Have someone give you unpublished holds right on top of a fix, read and understand the PDF they send you. Pay close attention to the Nav #1 and how it operates, three guys in my group had issues with it.
