You can also get it for free at the Delta GO, at the health clinic.

I received it in the military, but don't live local to my squadron. I've been having issues with local pharmacies not wanting to transcribe it over to the shot card even with my mil shot records.

They require new hires to have it before starting Indoc.

They are not technically employees yet so they would not be able to get it at the GO.

For the deployment I would just use the APO address, or you can list the unit and base. I doubt anyone at Delta will care.

I used the actual squadron street addresses for my regular assignments, but I'm sure just listing the squadron name and base is fine.
