I agree that a lot of guys got used to the flaps being at 20 even before they taxi out, but that's why we use checklists, to prevent us from missing something. Attempting to take off with the flaps up shows that the crew is not using checklists at all.
Here's my questions:
1) Taxi check: "Flaps" How are both pilots missing that the lever is set to up and the indicator is at 0
2) Before takeoff: "Hydraulics" How is the f/o missing that half of the pumps are not on
3) Line up: "Takeoff Config" Now this is the easy one. Is the message there or isn't it? If it ain't, don't go anywhere.
I'm sorry, but I just don't understand how we're still screwing this up. I heard we're up to 16 times now?

I wish I were as good of a pilot as you are.
For the record, I have not had this happen to me (attempting to takeoff with flaps at 0), but I have always found it extremely difficult and very arrogant to be critical of other pilots. You don't always know all the circumstances that lead to a certain error, failure, missed checklist, etc... Unless you have been in that situation or were sitting in the jumpseat when it happened, try not to be over critical.

AL - News) plan to terminate a contract with its Freedom Airlines Inc unit.
Phoenix-based Mesa said Delta notified it on March 28 of its intent to terminate a connection agreement with Freedom dated May 3, 2005.
It said Delta alleged that Freedom failed to complete a specified number of flights on ERJ-145 jets during three months of the six-month period ending Feb 2008. Mesa said the notice would allow Freedom to keep providing connection services to Delta while the airlines work out a "transition agreement."

Any video including Cockpit. I know of one crew that is in truoble now for posting the video.

Looks like our system chief pilot and new D.O. called in sick for 4 days (presumably off of a trip) last week. I wonder who he had to talk to when he called off?
