Would that be the pre-Covid normal, or the new normal?

Business or better is only on international legs longer than 3.5 hours right? So you could be in coach on a transcon?

Greater than 3 hours or any segment where the duty day exceeds 16 hours.

I think it doesn't apply to CONUS (Domestic, lower 48) though, correct....?

but a DH from Honolulu or Alaska to DFW would be bumped up from coach..... as it's not the 48 contiguous in CONUS.

It depends……..
Once you have a higher level of status on the airlines then upgrades are common. I was in economy on domestic flights 3 times last year but still in an exit row seat.
International travel is business class unless you opt for a payout and accept an economy ticket.

Youre correct. Appendix B adds quite a few foreign countries including Hawaii and Alaska to the list.

There are a lot of places outside of CONUS listed in Appendix B where the company is NOT required to provide Business Class travel. If you are a pilot here and are not familiar with that section of the CBA, you should read it.
