Let me be a little more clear, I think they are most worried about you having any failure in the 121 environment because of 3407. They brought up the subject on more that one occasion. If you have a failure from something like a CFI ride, I don't think that is as crutial, unless you have multiple pink slips.
I also do not believe a failed stage check in 141 counts as it is not on your permanent FAA records.

With all the talk about busted check rides for 121 carriers, I was curious, part of the paperwork asked about any unsatisfactory training experiences or checks. I went to a part 141 school and had a few "stage checks" or "phase checks", but none resulted in any FAA pink slip or notification, I have cleared several years of 121 check rides since, do these need to be reported and do they matter? I have heard several peoples different opinions about if I should disclose them or not. Any Ideas?
You are not required to report those, as even the FAA can't get that information. It's like your school transcripts.

Just because you have a couple of busts doesn't make you a bad pilot, but the regionals are shaking and cracking down right now and want guys with clean records. That's just the way it is.

I'm hoping you're right about that and after all, it was a looong time ago when I was a relatively new pilot compared to my experience nowadays.

You mean it hasn't been working properly? What new questions are you referring to. When I validate my app against the companies it says that I meet all the requirements. It says my app has been published to the targeted companies that I have chosen.

There is now. They changed the Addendum to include a flight history disclosure. The second question, asks about failed checkrides. They also added a Bachelor's degree to the requirements when you "compare the app". Both of those updates are very recent.

Commercial Pilots Certification with multi-engine and instrument ratings
Total Fixed wing of 1000 hours preferred
Multi-engine fixed wing time of 200 hours preferred
A current flying job (12 month experience)
Current FAA First Class Medical
IFR Currency
FCC License
Possess the ability to work in the US
Possess the ability to travel in and out of the US to all cities/countries served by
American Eagle
Ability to relocate
Ability to work weekends, nights, shifts, holidays, and overnight trips
Minimum age 21
* Flight time requirements are commensurate with experience
The following preferred requirements are common amongst Eagle’s most successful applicants:
Actual instrument time to be 10% of total time
Six months experience with a Part 121 or Part 135 operation
ATP Certificate or successful completion of valid ATP written test
Hours flown in most recent 12 months to exceed 100 hours
Fixed wing multi-engine turboprop experience
