Safety has nothing to do with Section 18s. Flight Ops Management wields the, “big stick” in most airlines and PSA is not immune to that. So when the executive team tells them to “fix” something they have a limited number of tools available to them in their tool chest. Hence the disciplinary hearings.

Just wondering if the commute to DCA, PHL, DAY, and CLT is pretty easy from DFW? Looks like a lot of flights on American to those locations. I would like to stay in Dallas and commute. I know life on reserve would be better in your domicile. However, really like the metroplex.

but.. This is obviously a systemic problem. It happens over and over and over again with lots of different crews. One solution is “how do we change the system so that the problems stop happening?” The other option is “lets keep the same system but threaten the people who operate it.” The latter choice is sometimes effective, but the former one is the better way to run an airline.

There's a couple of things here though - it hasn't been a problem for XX years, it's part of the Acceptance and Before Start flow. Bottom line, people are not doing their job. If they were doing their job, then it wouldn't be happening. Hence, drop the hammer. That's how it works in the theoretical world anyhow, I'm with you that we need to know why the people that forgot, forgot, what lessons we can learn from it and improvements we can make.
Just a thought - forced upgrades, lots of younger people taking the helm. What I've seen from my FOs is that they are always in a rush ! Always wanting shortcuts and flying fast. Impulsivity is one of the five dangerous attitudes. Eh, my two pennies worth...

The rumor I’ve heard is that the problem is not a change in crew behavior but a change in inspection guidance from Mitsubishi that makes this expensive.
But I will stand by my assertion that when lots of meat servos make the same mistake over and over again, the long-term solution is to change the system not beat the meat

I don't disagree that there is always room for improvement. I'm actually the Safety guy at my Guard unit for whatever its worth.
