I have an important personal commitment 4 months after my class date where I will need a long weekend off (Fri-Sun Minimum). Missing the commitment is not an option. Should I:
(a.) Tell management on day one of class
(b.) Wait and see what schedule I am awarded before mentioning it.
(c.) Hope that I will have enough PTO to use for any trips/reserve that are in conflict.

Luckily... There's no probation period here.
Can't help,when you get ill. Physically or mentally. IMSAFE.
I'd hate to have you online not able to devote 100% of your attention to flying safely. That would be a violation of 117. You'd be unfit for duty. Best to take the weekend off for your own well being.
Btw... A visit to the chief pilot isn't necessary. Just call crew services.
"Hello this is F.O. DJ, number 12345. (How can we help you today) please put me down for PTS (or UTS) for my reserve period thanks" ok have a nice day.
Who knows maybe... You'll get it off anyhow?

If it's something like a kid's graduation or a wedding, just do everything you can do to get that weekend off...try to drop, swap, request it off, etc. IF THAT ALL DOES NOT WORK, then talk to your Chief Pilot. Most likely, they will put a LUV tag on it and ask if you want it paid or unpaid. They have been pretty good about that. DO NOT call in sick (for many reasons.)

I can't think of one reason not to call in sick if your not feeling well enough to fly. The DP isn't a reason either, it's been going on since long before it was in writing.
